Patients: To make a new appointment in Dr. Sanger's pediatric movement  disorders clinic, please contact the Harman Clinic at Packard  Children's Hospital, 650-724-4270 (fax 650-724-5344).  We will need a consultation  request form faxed to us by your primary physician.  Please bring  copies of all medical records, laboratory results, and other testing  results to your appointment.  Please also bring copies of any MRI or  CT scans (the actual films, not just the report).  The clinic is  located at 730 Welch Road, Palo Alto, California (Google Maps).
