| Protocol Information
The regular form divides the Protocol Information
section into five subsections. Depending on your answers in the procedures subsection, two additional sections may be added - Pain/Distress Categories and Procedure Relationships.

You may move through the Protocol Information section using the page icons at the top of each page, or by clicking the Next and Previous arrows at the top and bottom of each page.
Rationale and Procedures
The Rationale section is a general questions section. These questions will be very familiar if you've used the old Prism APLAC system. This is where the background information for the protocol is listed.
The Procedures section is a little more complicated. For each procedure, use the Add button to add each procedure. There's no limit to the number of procedures you can add to a single protocol. To practice adding procedures, click image below.

The most important thing to remember about the Procedures section is to be sure to complete the additional subtabs after the initial save of information. These subtabs vary in number and type depending on the type of procedure. If your procedure does not meet any of the types specified, choose Other. Many procedures fall into the Other category.
Conditional Sections
The Pain/Distress Categories section and the Procedures Relationships section appear based on your responses in the Procedures section. It's important to fully complete the Procedures section before finishing your responses in these categories. The Pain/Distress Categories section appears only if you select a pain category of D or E. Different questions are required for the two different pain categories. The Procedure Relationships section appears when you have more than one Procedure listed. In this section, describe the relationships (or lack thereof) between all of the procedures you listed.
Husbandry
In the Husbandry section, you only need specify special conditions such as restrictions on food or water or cage size. If nothing is specified here, then it is expected that the animals will experience normal husbandry conditions.
Euthanasia
As you specify how the animals will be euthanized, please note that depending on the method, you will be asked varying follow-up questions. If you choose a physical method, you must specify whether it will be anesthetized or not. If the animal is anesthetized, you must specify the drug and dosage for the anesthetic. If unanesthetized, you must provide a justification. For non-physical euthanizations, you must specify the drug, method of administration, and dosage.
Attachments
Other attachments are added in the last section.
You can add grants, explanatory diagrams, a table of animal usage, or any other
materials that will be used in association with your protocol.

Browse to find the document you wish to attach.
Once you select the document, the current name of the document will
default into the Document Name field. You may alter the name to
anything you choose. This will not affect the name of the document
on your computer. The Document Name you choose will display on the
Attachments page.
Once your protocol information is entered, you may
wish to view and print the protocol. The next section will show
you how to do this.
