MTAs

Instructions for Obtaining Reagents:

Follow the steps below or we can not process your request. All incomplete requests will be set aside and not be processed!

STEP ONE: To start the approval process, complete the online form. All requests for reagents must be approved first.

Be sure to complete the entire form. Answer all questions. The form will ask for your method of payment of $50.00 for shipping and handling. You MUST include the one paragraph description of the intended use of the requested reagent.

STEP TWO: Wait for approval.

We will respond to your request ASAP. If there has been no response after three weeks, contact Kassie Koleckar , Lab Coodinator. Contact Kassie for any questions regarding shipping and wire transfer information.

STEP THREE: Download and complete the Materials Transfer Agreement (MTA).

This MTA is for use by academic or non-profit institutions. Corporations must contact the Office of Technology Licensing at Stanford University

The MTA must have the name, SHIPPING address ( a street and number) and phone number of the responsible person and a signature of the Principal Investigator of the Lab. The MTA can not be signed by students or postdoctoral fellows.

We do not require an institutional signature. However, if your institution requires it's own institutional signature, please inform them of the following: This agreement is NOT a product of Stanford's Office of Technology licensing. All issues concerning this agreement are handled by Dr Helen Blau's office so have them contact us directly. Also, we are not well- equipped to handle requests for individualized MTA's — such requests will greatly delay the processing of your request. The only other MTA that we are accepting is the Uniform Biological Material Transfer Agreement (UBMTA), is your institution is signatory to that agreement. The terms of our standard MTA are not intended to be more restrictive that the UBMTA.

Attach a cover letter to the MTA with a one-paragraph description of the intended use of the reagent.

Include payment, either a check for $50.00 (U.S. funds only) made payable to Stanford University, or send a wire transfer, to cover the costs of preparation of materials. We can not accept credit cards.

SHIPPING INFORMATION: If expediated shipping is desired, please include your FEDERAL EXPRESS account number on the MTA to charge for shipping. Otherwise, standard US Postal service delivery will be used. There is no extra charge if US Postal service is used.
