Please note: our Forms page has changed slightly; additional forms and instructions are now available.
In order to qualify for services, you must be registered with the OAE. Visit the Initiating Services Page to begin the process. If you are already registered, you can submit any of the following forms for processing by the OAE. If you are not sure whether or not you are registered, contact our office at:

The service request forms below are referred to in Section V of the Policy and Procedure for Student Requests for Services and Accommodations. That section also includes recommended timelines for requesting accommodations, auxiliary aids and services. If you need any of these forms in an alternative format or require assistance with completing the forms, contact the OAE. Please let us know immediately if there is a delay or problem with your accommodations, services or auxiliary aids.
Forms marked "Online" may be completed and submitted online. Forms marked "PDF" may be downloaded and completed on your computer, then submitted as e-mail attachments. You may also print out a completed PDF form and submit it in person.
To submit an ONLINE form:
(1) Click on the name of the form or application you want to submit.
(2) Fill out form -- be sure to complete all required fields.
(3) Click the "Submit" button. You will see a confirmation screen.
To submit a PDF form:
(1) Right Click (Windows) or Control+Click (Mac) the name of the form or application you want to submit.
(2) Select "Save file to Desktop" (Windows) or "Download Linked File" (Mac) .
(3) Open file in a recent version of Adobe Reader. (The latest version of Adobe Reader can be downloaded for free from Essential Stanford Software.
NOTE: Do not open using Apple/Mac Preview or Windows Picture and Fax Viewer, otherwise the PDF form cannot be filled-in and saved properly.
(4) Fill out form -- be sure to complete all required fields.
(5) Click the "Save Form" button at the bottom, and save to your Desktop.
(6) Email the completed form as an attachment to the appropriate coordinator (shown on form).
