Form used by a Judicial Panel only in a situation when a reporting party -- in the face of the Judicial Officer's discretionary decision not to charge -- has requested an evidentiary hearing to determine whether there is in fact sufficient evidence to formally charge a student with a violation of the Honor Code, Fundamental Standard or other University policy.
