Photocopy of current valid I-20
Completed degree conferral form (signed by advisor or dept administrator) with projected completion date marked.
Completed and signed PTA form:
Level of education being sought: BS, MS, Engineer, PhD, etc. Must match degree level designation on students I-20.
Students major field of study. Should be same as department listed on I-20.
Describe proposed employment. Even if student does not yet have job offer, a description of some "projected" job relevant to the students field of study must be indicated.
Beginning/Ending dates: beginning date cannot be prior to projected completion date listed on degree conferral form and cannot be later than 60 days after that date. Ending date can be up to one year after start date if no previous periods of optional practical training have been taken. Otherwise, the amount of time taken during previous periods of optional and post-completion practical training must be calculated and deducted from the total 12 months allowed. Even if student does not yet have job offer, the beginning and ending dates must be filled out.
Number of hours per week must be indicated as full-time.
Dates of previous periods of practical training should be listed in section #9. Each period listed should be specified as either curricular or optional and either full-time or part time.
Form must be SIGNED and dated
