Visitors, lecturers and others who are to receive an honorarium or similar payment through a check request from Travel and Reimbursement must have a Social Security number or other tax ID number before the check will be issued. Click here to open a Word document with an ITIN and 8233 "signature-ready" application form. The form may be partially completed prior to your visitor's arrival; provide the missing information prior to your visitor's departure. The I-Center will make signature-ready IRS forms available for your visitor's signature.

Do not ask your visitor to complete the W-7 form itself. We cannot process applications comprising IRS forms that have been filled out by hand or incompletely. A $100 surcharge will be assessed if we are required to follow up an incomplete or illegible ITIN application.
