University Loan Forms

Students who wish to borrow University loans (institutional loans) must complete a Self-Certification form and must review an Application and Solicitation Disclosure Form. To access these forms, please follow the link for your school/program:

Step 1:  View Loan Application and Solicitation Disclosure

Please follow the link for your loan type as shown on your financial aid award letter in Axess. Please carefully review the Application and Solicitation Disclosure form. You may print or download a copy for your records.

Step 2:  Print, Complete, Sign and Return Loan Self-Certification Form

Please follow the link below to access the blank Loan Self-Certification form. Print the form, then complete all fields and sign at the bottom.  Return the completed, signed form to the Financial Aid Office at Montag Hall, 355 Galvez Street.  You may also fax the signed form to the FAO at (650) 725-0540.
You must complete this step before your loan funds can be disbursed to you.
 

For Section 2, items A and B, you can find these values on your financial aid award letter. The value for item A is shown on your award letter as"Total Expenses."  For item B, subtract the amount of your institutional loan from the amount shown for "Total Financial Aid."
