CS-FWS Authorization Request

Instructions

This form is for undergraduate students applying for Community Service Federal Work-Study (CS-FWS). Please complete all fields below. When you have finished, click Submit.

Your information will be sent to the Financial Aid Office (FAO), where your eligibility for CS-FWS will be determined. Within one week, the FAO will notify you and the Haas Center by email of your maximum earnings amount. The Haas Center will not process your CS-FWS application until this step is completed.

NOTE: The Community Service Federal Work-Study program for Summer 2009 has reached full capacity. We regret that we are unable to process any more requests for Summer 2009.
