The Stanford University Proposal Routing Form (SU-42) is the tool Stanford uses to collect the information it needs to submit a sponsored project proposal. It is also the form used to document the necessary approvals of the proposal.

    NEW

In May 2007, the Office of Sponsored Research introduced a
NEW SU-42 form for the entire campus. The form, along with instructions, samples, and other related information is on the ORA web site at

Proposal Routing Forms

SU-42 FORM  NEW

All schools are now using a common SU-42 form, provided by OSR as a downloadable Excel file at http://ora.stanford.edu/ora/osr/proposal_development/su42_07.asp

The only proposals that will NOT use this new form are those for Fellowships. For Fellowship applications, use the form provided by the School of Medicine at http://med.stanford.edu/rmg/fellowships.html
